Minnesota Careers:


Personal Care Attendant / Home Health Aide
Ongoing Open Position 8/08 : All Minnesota Locations

Personal Care Attendant/Home Health Aide will perform direct client cares and will assist clients in activities of daily living.

Major Responsibilities/Position Functions:

Assistance in accomplishing activites of daily living:
1. Dressing
2. Grooming
3. Bathing
4. Eating
5. Toileting
6. Transferring
7. Positioning
8. Mobility
Redirection and intervention of behavior including observation and monitoring.

Physical Requirements:
Must be able to lift, turn, position and transfer up to 30 pounds of weight.

Education/Qualifications:
Individual must have high school diploma or equivalent and be at least 18 years old (or if 16 to 18 years old, has participated in a related school-based job-training program or has completed a certified home health aide competency evaluation).
Individuals must show compassion and empathy of persons who are chronically ill or otherwise disabled, and demonstrate an understanding and maturity with respect to the responsibilities and expectations of the position.

RN or LPN
Ongoing Open Position 8/08 : All Minnesota Locations

Private Duty Nurse RN or LPN

The Private Duty Nurse works under the supervision of the Nursing Supervisor to provide direct client care as directed by the client's physician and specified in the plan of care for each individual. The Private Duty Nurse uses skilled nursing judgement and technical support to insure that services are delivered in accordance with professional nursing or medical standards.

Major Responsibilities/Position Functions:

A. Clinical
1. Implement nursing cares as indicated in the client's plan of care and according to professional nursing standards which includes administration of intravenous medications.
2. Assess client condition continuously reporting and documenting significant changes in the client's condition.

B. Administrative
1. Maintain accurate and comprehensive client medical data.

C. Miscellaneous
1. Other responsibilities and duties that may be periodically assigned by the Nursing Supervisor.

Physical Requirements:
Must be able to lift, turn, position and transfer up to 50 pounds of weight.

Education/Qualifications:
Must be a graduate of an accredited school of nursing and licensed in the State of Minnesota.

PCW and HHA/CNA

PCW and Home Health Aide/Certified Nursing Assistant will perform direct client cares and will assist clients in activities of daily living.

Major Responsibilities/Position Functions:
