Using the latest technological support in home care, REM Health can provide its clients immediate access to their nurse through the Telemedicine program. This is particularly helpful for clients who require frequent nursing, clinic or ER visits, clients who are anxious or non-compliant, or those lacking support systems. Individuals with Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, Diabetes, Dementia, Cancer, and Anxiety Disorders can especially benefit from using Telemedicine.

Telemedicine has proven instrumental in helping individuals live at home longer because it is a more effective means of monitoring the client's physical condition. Hospitalizations due to exacerbation of the client's condition can often be avoided or diminished.

Telemedicine gives the client an opportunity to take an active role in their care, which helps to build confidence and independence.




REM Health can perform these assessments using Telemedicine:
* Blood Pressure
* Weight
* Heart and lung sounds
* Bowel sounds
* Blood glucose readings
* Wound status
* Oxygen concentration (oximetry)

Assessment results can be tranmitted to the physician. Telemedicine is covered under the Medical Assistance and Alternative Care waivers
